Breathing. This is the first exercise I give to almost every patient I see. Specifically, I train them in diaphragmatic breathing, also sometimes referred to as “belly breathing”. In my second post, the pelvic floor is only part of what makes up our core - the other two parts are our abdominal muscles and our DIAPHRAGM. 
The picture above shows that the diaphragm is the ceiling of our core. In this position, it functions in opposition to the pelvic floor muscles. As we inhale, our diaphragm descends into the abdominal cavity so that our lungs can expand, effectively forcing a stretch of our abdominal wall and our pelvic floor muscles. Mechanically, this facilitates normal mobility and coordination of these muscle groups.

Diaphragmatic breathing also benefits our system physiologically. You have likely heard the term “fight or flight” in reference to our sympathetic nervous system (SNS). With the bright lights, loud noises, and long to-do lists that plague our days, many people have lives which are ruled by the “fight or flight” system. If your body is run by your SNS, you will experience increased heart rate, increased respiratory rate, increased blood pressure, as well as symptoms often addressed in pelvic floor physical therapy like chronic pain, poor digestion, and sexual dysfunction. With diaphragmatic breathing, you can break the cycle of “fight or flight” and learn to spend more time functioning from the “rest and digest” or parasympathetic nervous system (PNS). How do you know if you are breathing with your diaphragm? First, place one hand on your chest and one hand on your belly. As you inhale, you should feel your belly expand like a balloon with minimal movement felt at your chest. As you exhale, you should feel both hands return to their starting position. It is important to note that you are not forcing your belly to expand or purposefully taking deep breaths. We are only changing the location of the breath, not the rate or the depth. The first video demonstrates “chest breathing” while the second video demonstrates correct “diaphragmatic breathing”..

The pelvic floor is a muscle group in women AND MEN that creates the base of the pelvic girdle. There are fifteen pelvic floor muscles, plus a couple of hip muscles that are often included as well. Their function is four-fold:

Supportive: to keep your insides inside of you. There is a normal, constant, low-level engagement of the pelvic floor to keep the genitourinary, reproductive, and gastrointestinal organs supported. When this function is impaired, women may experience vaginal prolapse, and men and women can experience rectal prolapse.
Sphincteric: to keep urine and stool inside of you via muscle engagement, and allow for voiding in the bathroom via muscle relaxation. Decreased muscle activity can result in urinary or fecal urgency and incontinence, while increased muscle activity can result in urinary hesitancy (“a shy bladder”) or constipation.
Stabilizing: to participate in core stability. The fitness industry has been promoting the importance of “the core” for years. Often this fad is focused solely on the strengthening of the abdominal muscle group. However, the core is a closed system with the diaphragm muscle as the ceiling, the abdominal muscles as the walls, and the pelvic floor muscles as - you guessed it - the floor. When these three muscle groups are not working in coordinated function, symptoms such as chronic low back or hip pain can result. Common antepartum symptoms like pelvic pain during pregnancy and diastasis rectus abdominis (DRA) are also an effect of core muscle dysfunction.
Sexual: to allow for pain free and pleasurable sexual participation in people of all sexes. The pelvic floor muscles must relax for vaginal or anal penetration. If a person is experiencing pain with penetration - whether it is with use of internal menstrual barriers, a speculum at an OBGYN exam, or something for pure sexual function - it is likely that these muscles are incapable of releasing tension appropriately. Additionally, the pelvic floor muscles must have adequate strength and coordination for female orgasm, or male erection and ejaculation..

Understand your body and learn what is normal for YOU. Seek appropriate care when things change. For example, some vaginal discharge is normal when it is white and relatively odorless. A discoloration from your normal and often accompanied by an odor is a sign you should have checked immediately. Look at your skin and the tissue in the area — any changes you notice over time?  The person that cares most about your body should be YOU! Know yourself and say hello down there 👋🏻

Should you abstain? I get this question ALL. THE. TIME. In short, we are all adults and we are going to do what we want! Will having painful sex set you back while you are rehabilitating? Perhaps a little. Will it permanently set you off course? Absolutely not. But it’s not helping you or your partner if it is getting worse over time and / or emotional distressing to you. You must find a pelvic floor physical therapist and team of providers that can get you set on a healing path 🌟

Most people have heard “I couldn’t get it up!”, referring to male #erectiledysfunction 🍆 but its pretty uncommon to hear from a women’s perspective “I couldn’t get it IN”. ❌🍩

Vaginismus is a kind of #sexualdysfunction. Vaginismus is a condition in which there is involuntary contraction of muscles around the opening of the vagina with anticipation of any sort of vaginal penetration (tampon, finger, penis etc). So any time there is any sort of attempt to penetrate the vagina the body and brain send a signals to your pelvis essentially saying “#NOPE” - causing your pelvic floor muscles to tighten up. 😳 
Thats not to say the pain is in your head, but really more of your body’s way of going into a reflexive state of protection, because of past experiences. (Kind of like when your eyes close reflexively 👀when something gets close to your face😫).

There is no one cause of vaginismus; There are emotional/ #psychological factors like anxiety, stress, past #sexualtrauma or assaults, fear of sex, underlying religious or cultural connotations associated with sexual expression etc. 
Physical factors such as cysts, #childbirth, #pelvicsurgeries, or multiple infections can also be causes of vaginismus. 🎊GOOD NEWS🎊: you can get help with this!! There are amazing pelvic health PTs, sex therapists, mental health professionals who work with women with vaginismus to get them where they need to be. Finding that #tribe of people to help you recover and advocate for you is HUGE. ❤️👯‍♀️❤️
